A written application by a requesting person to transport the remain either to the embassy of Ethiopia in Brussels, or to the Ministry of Foreign Affairs of Ethiopia in Addis Ababa. The application should indicate that the expenses be covered by the requesting party. It should also include the name and address of the receiving person in Ethiopia.

• Original and a copy of Death Certificate.
• Original and a copy of letter from the funeral home showing
embalmment;

• Original and a copy a letter issued by local authorities that the body is fit for transportation;
• Laissez-Passer for Corps issued by a of Non-contagious disease letters issued by the local health authority and/or Department of Health
• Security Certificate showing that the coffin is sealed in casket;
• Passport (Any Travel Document) of the deceased;
